Rail Accident Investigation Division (RAIB), an independent body that investigates accidents to enhance railway safety, reviewed an incident involving an Avanti West Coast train traveling from Glasgow to London Euston. The incident occurred at Euxton Junction on the West Coast Main Line, where a controller of site safety (COSS) was performing scheduled maintenance work near the tracks.
The COSS, tasked with locating a lineside equipment cabinet (LOC) for maintenance, failed to identify the correct line blockage, leading to a close call with the passing train. Despite signage indicating the layout of the tracks accurately, the COSS misinterpreted the information and was not corrected by another team member during a briefing.
The incident took place near Euxton Junction, south of Preston, where four railway lines converge. The lines involved in the incident were the Up Fast and the Down Fast lines, with two additional lines designated as the Up Slow and the Down Slow.
The COSS received a work package containing safety instructions (SSOW) but had already begun work before reviewing it, violating Network Rail safety standards. The work package included instructions for maintaining a safe distance from the tracks and designated line blockages for specific tasks near the tracks.
The team was assigned two maintenance tasks within the same area: testing within an LOC and a track circuit inspection. Despite being familiar with the location, the COSS mistakenly identified a different LOC near the Down Fast line, leading to confusion about the actual work location near the Up Fast line.
As a result, the COSS left the depot with the incorrect assumption that the work was to be carried out near the Down Fast line. This misunderstanding contributed to the close call with the passing train.
